Please Complete a Survey for the Onondaga County Health Department

The new school year will begin for SCSD students in elementary school, middle school, and grades 9-10 on Wednesday, September 6th and for grades 11-12 on Thursday, September 7th. Did you know that all children are required to be up-to-date on certain immunizations in order to attend school? The Onondaga County Health Department (OCHD) wants to help you ensure that your child has the vaccines they need to attend school this year!
 
To help the OCHD close gaps in access to vaccines, they are asking for your help.

This survey will help the OCHD staff understand the challenges that parents and guardians face in getting their children vaccinated for school. The Health Department will follow-up with respondents to help coordinate next steps.
