Students attending school are required to have a physical exam prior to entering kindergarten, 2nd grade, 4th grade, 7th grade and 10th grade as well as to be cleared to play sports and to obtain working papers.  All physical exams must be done through your own primary care physician.  The Syracuse City School District’s Office of Health Services will not be administering any physical exams for students.

If you child attends a school with a school based health center (Dr. Weeks, Dr. King, Delaware, Franklin, Grant, HW Smith at Levy, WSA at Blodgett, and Fowler), you can register your child in the center and they can get their physicals during school hours. All children who attend these schools are eligible for services from the SBHC.

If your child is in need of a physical and you do not have primary care physician and/or do not attend a school with a SBHC you contact one of the following sites, read more here.
